Because we know your time is valuable, we adhere to the appointment schedule as closely as possible. Because emergencies do arise and will complicate the best of schedules, we ask your understanding at these times. Should we see that we are unavoidably running late, we will advise you of the situation and give you the option to reschedule your appointment. When making an appointment, please give the receptionist the reason for your visit so that we can schedule the proper amount of time for your child. Since some pediatric evaluations take more time than others, this will help us to serve you better. Call as early in the day as possible, if you have an immediate need because of an injury or illness to have your child seen for a same day appointment. Out of consideration to other patients and to keep to our schedule, children without an appointment will not be seen at the time of a sibling’s appointment, unless you call first to schedule. Your cooperation in this matter is appreciated as it will allow us to see patients in a timely fashion as we do value your time. For most acute illnesses, please keep in mind, you will be offered the first available appointment with whichever provider is in the office. Well-child visits and/or consultations (for ADD, diabetes, etc.) may be scheduled with your preferred physician or nurse practitioner. This scheduling system provides you with total and personal attention in the care of your child. For non-emergency care, minors should be accompanied to the office visits by a parent. If a parent is unable to accompany the minor, a written consent for treatment (TEMPORARY MEDICAL RELEASE FORM) must be provided to the office. A parent should accompany their child to all well-exams when an immunization is to be given, because they need to sign for all immunizations until 18 years of age by law. NO EXCEPTIONS CAN BE MADE. Give us at least 24 hours notice if you can’t keep your appointment or, if you’re running late, please give us reasonable notice. Please help us serve you better by keeping your scheduled appointment. If you arrive more than 15 minutes late after your scheduled time, and didn’t call to let us know, you may be asked to reschedule your appointment. Our policy is to charge for continued missed appointments as per OUR FINANCIAL POLICY. Physical forms for sports, camp, school or daycare, work forms, and insurance forms will be completed, whenever possible, within 3-5 working days. Often, a physical exam may be needed if your child has not had a well-exam within the last one year unless stated otherwise. For DRIVER’S EXAMINATION FORMS, the patient must come to the office to complete and sign the form.
